In compliance with Washington State law, health insurance carriers must now offer alternative coverage to their insured. We are currently preferred providers for Regence Blue Shield and the American Whole Health Network (Aetna, Cigna, FirstChoice, and NACBN). We also provide Washington State Department of Labor & Industries (L&I) insurance billing and your own personal PIP insurance.

We are preferred providers for Aetna, Cigna, First Choice, Labor & Industries, Blue Cross/Blue Shield, and many others. Medical insurance companies require that you get a prescription from your primary care physician, which is theonly way you can prove medical necessity for treatment.
Prescriptions should included a diagnosis and a time requirement. An example of this would be "Neck sprain" and "Massage for 2 times a week for 6 weeks. "Some of these insurance companies require you to get a referral from them instead of a prescription from your doctor in order to be covered for treatment.
You will be required to make your copay and any coinsurance payments at the time of the appointment. Please call the telephone number on the back of your insurance card for questions on your individual policy.
If Seattle Massage Center is not considered a preferred provider for your insurance company, check if you can self-pay for treatment from an "out-of-network provider" and then submit your payment receipts to the insurance company for reimbursement afterward.

| Personal Injury Protection (PIP) |

If you own a car and have car insurance, you have personal injury protection (unless you have personally signed away that provision on your policy).
If you have an accident and it is your fault, your insurance company will cover your medical needs. You will need a prescription from your doctor, a claim number and the name of your adjuster from your personal car insurance company.

If the accident was the fault of the other car, that is called "third party coverage." We do not bill for third-party coverage. However, please call your car insurance company and ask if they will cover your expenses and mediate with the other insurance company for you. In that case, we will bill your auto insurance for your payments. If they will not do this for you, you can pay at the time of treatment and seek reimbursement.

| Labor and Industries (L&I) claims |
|

|
If you are injured on the job, you are entitled to medical coverage from your employer. You must first fill out forms at work, get a claim number, notify Washington State Department of Labor and Industries and get a prescription from your primary care physician stating diagnosis and length of time required for treatment. If your doctor is unsure how long it will take, a good measure is to request 6 massages and reevaluate.

We are happy to write reports to keep your doctor informed of your progress. Labor and Industry will pay for 6 visits at a time and then require communication from your massage practitioner for further treatment.

| How long is a treatment session? |
|
|
|
|
Massage therapy sessions are invariably an hour long and may involve time taken to assess the state of an injury or condition and discussion regarding treatment and self-care. Acupuncture sessions are also typically an hour long, but your very first visit will be 90 minutes to accommodate assessment of your state of health and your injury.

| We require at least 4 hours advanced notice if you wish to cancel or reschedule your appointment. Please call more than 4 hours before your appointment if you need to cancel or reschedule to a different day. Those not doing so will be charged the full rate for any missed appointments or late reschedules. |

Clients arriving for their first visit should sign in at the front desk between 10 and 15 minutes before the start of the appointment in order to fill out a short health intake and several health insurance forms.
It is advisable to arrive early enough to deal with traffic and also with parking, which can be scarce on nice days due to the influx of visitors to the lake.
